Client Registration Form (separate to online enrollment)
Please complete the Client Registration Form prior to your training session.  It can be found here, and you can email it to perfectpoochaz@gmail.com, or you can print it and have it ready for you trainer at the time of your session.  Please call or text with any questions.  

Bookings and Fees
You are able to book a Sunday initial 2-hour behavioral session (new clients), or a 1 or 2 hour follow up here (previous clients).  Once you have booked your time, we will send you a confirmation via text/email.  DON'T FORGET TO CHECK FOR ANY TRAVEL FEES HERE BEFORE BOOKING AS THEY WILL BE ADDED ON AT TIME OF SERVICE.  Please call with any questions. 

Waiting List
If you don't see any dates that work for you, or there is an extended wait, book a session that works for you and then go back into the booking system, select the service you want, and then select "Waiting list" at the bottom and fill in the timeframe that works for.  You will automatically receive a notification if a timeslot opens up in that timeframe so you can reschedule to that date.
